When offline, submitted data will be stored locally. When online, locally stored data will be resent to the server.

Walk around and inspect the vehicle

Is the vehicle 'Red Tagged' i.e. has a safety defect?*

If 'Red Tag' is No, scroll to the bottom of the form and 'Submit'

If 'Red Tag' is Yes, please select an option from below, before submitting the form

Are there chips/cracks in the windscreen above the steering wheel (Zone A) greater than 10mm, or elsewhere (Zone B) greater than 40mm? Are the windscreen wipers missing, or damaged (rubber loose or falling off)? Are the side mirrors missing, cracked (hindering driver vision) or loose (hanging off vehicle)? Are body parts loose/missing? Are light covers broken, exposing bulbs, missing or reflectors not intact? Are indicators broken (not flashing)? Are tyres bald (<1.6mm - check with tyre gauge), have foreign objects (nails) in tread, have cuts or bulges to sidewalls?
